	(a) An incorrectly fitted cover plate. In this case the aerofoil shaped cover has been wedged against a glass fibre pressure plate of inferior quality.
	(b) Roller wave distortions in toughened glass. This is a consequence of the manufacturing process, which can cause optical distortion for occupiers in extreme cases. Glass manufacturers are constantly working to improve processes and quality management to eliminate this problem.
